Each year, schools must provide to enrolled students a notice containing a list of the consumer information it must disseminate, and the procedures for obtaining this consumer information. Schools must also provide a notice of student rights under the Right to Know and Family Educational Rights and Privacy Acts. The primary method of disseminating consumer information to students is through the college catalog, student handbook, and guide to financial aid. In addition, information is also distributed via electronic email, and the SWC website.

Southwestern College is accredited by the Higher Learning Commission of the North Central Association of Colleges and Schools. The Art Therapy/Counseling Program is also approved by the American Art Therapy Association.

Bookstore

The SWC Bookstore is in the Enrollment Services/Administration Building. Textbook lists are compiled a quarter before and are made available for purchase at the Bookstore. Students can find the ISBN and price of each book on the Quarterly booklist, which is made available on Populi. Students can purchase their books through the SWC Bookstore using their financial aid if the student has funds to cover tuition, fees, and the books.

Student Support Services coordinates campus-wide efforts to provide services and accommodations for students with disabilities. In compliance with the Americans with Disabilities Act (ADA) and the Rehabilitation Act of 1973, Student Support Services ensures that eligible students receive proper classroom modification and serves as a liaison between faculty and students. Medical or psychological documentation is required to recognize a disability. More information about disability services policies at SWC is in the Catalog and Student Handbook pages 76-78.

The Financial Aid Office aids and counsel’s students in completing the Financial Aid Application, entrance and exit loan counseling, PLUS Loan applications, Scholarship applications, and financial aid verification for agencies. Withdrawal Policy

Withdrawal from the college means that you do not plan to return for at least one year. Withdrawals from the college may be requested by students due to health, medical, family, or personal issues. The SWC Withdrawal Policy is located in the Student Handbook, Part 6: Academic Policies Pages 53-55.
Refund Policy

Southwestern College provides refunds to students who are enrolled and dropping any courses. Refunds of 100%, 90%, 75%, 50%, and 25% are given based on the scheduled refund policy. More information about the refund policy is available in the student handbook and the Office of Financial Aid Policies and Procedures Manual.
Return of Title IV Funds Policy

In accordance with the 2008 Reauthorization of the Higher Education Act of 1965, SWC uses the “Return of Title IV Funds Policy”. This policy is available in section 12 of the Office of Financial Aid Policy And Procedures Manual.
Alcohol and Drug Free Campus Policy

The Drug Free Schools and Communities Act Amendment of 1989 requires the College to certify to the Department of Education that it has adopted and implemented a program to prevent the illicit use of drugs and the abuse of alcohol by students and employees. SWC is in compliance with the requirements of the Act. The Alcohol and Drug Free Campus Policy is available in the Student Handbook page 22.
Campus Safety and Annual Security Report

To assure compliance with the Student Right-to-know Act and the Cleary Act, SWC publishes the Annual Security Report of Crime Statistics. The full report may be accessed at: https://www.swc.edu/frequently-asked-questions/security-and-safety-statistics-for-swc/. The report contains statistics of occurrences of crimes such as murder, sexual offenses, burglary, and hate crimes that have occurred on campus as well as statistics for the number of arrests made on campus for liquor law violations, drug abuse violations, weapon possessions, and hate crimes.
Family Educational Rights and Privacy Act of 1974 (FERPA)

In accordance with the Family Education Rights and Privacy Act of 1974, as amended, persons of any age who attend a post-secondary educational institution that receives federal funding are hereby informed of the right to inspect and review their official education records. SWC considers attendance to begin when a student is officially accepted or conditionally accepted. Students should submit to the Registrar or Financial Aid a Release of Information Form. An annual FERPA informational email is sent to students via email.
Code of Student Conduct

Student conduct in the environment of an institution of higher learning is expected to be exemplary always. The regulations listed within the Code of Student conduct is promulgated by the Office of Student Support Services. The administration of SWC has delegated and authorized the Director of Student Services as the office responsible for the administration of discipline. The Code of Student Conduct is available in the Catalog and Student Handbook, pages 83-89.
Completion/Graduation Rates and Retention Data

Graduation and retention rates are available through the Registrar’s Office. These statistics are published in the annual IPEDs report and are disclosed to all students after submitting a FAFSA.
Title IV Loan Counseling and Master Promissory Note

Prior to receiving the first disbursement of a Direct Unsubsidized Loan, the student must complete entrance counseling. In addition, all student borrowers of a loan made under the Federal Direct Loan Program must complete exit counseling after the borrower ceases to attend at least half-time enrollment at the college.
Entrance counseling and the Loan Master Promissory Note (MPN) should both be completed online at www.studentaid.gov. Both must be on file and tracked in Populi before funds can be approved and sent to SWC. This insures that the student fully understands their rights and obligation as a student loan borrower. Entrance counseling informs students of their various loan repayment responsibilities including repayment options, interest accrual, and aggregate loan limits.
Once the student has graduated, dropped below half-time enrollment, or has withdrawn from school, the Office of Financial Aid will send the student notification to complete Exit Loan Counseling via email. Exit Counseling informs students of their various loan repayment responsibilities and options.

As a recipient of financial aid, there are certain rights and responsibilities of which students should be aware of. These rights and responsibilities of students on financial aid are listed in the yearly Guide to Financial Aid, the NASFAA Student Index, and the Federal Student Aid website.

Student’s Responsibility

- Keeping the Office of Financial aid informed of any changes in address, name, marital status, financial situation, or any changes in student status
- Reporting to the Office of Financial Aid any additional assistance from non-institutional sources such as scholarships, private loans, fellowships, Veterans Affairs educational benefits

- Maintaining Satisfactory Academic Progress
- Re-applying for the FAFSA each aid year

Constitution Day

The school holds educational events pertaining to the United States Constitution at the Office of Financial Aid on September 17th of each year. This event includes fee informational material, fun activities, and constitution booklet handouts. Students are notified of Constitution Day activities via email each year.

Private Education Loan Certification

Pursuant to Section 155 of the Higher Education Act of 1965, as amended, and to satisfy the requirements of Section 128(e)(3) of the Truth in Lending Act, a lender must obtain a self-certification signed by the applicant before disbursing a private educational loan. The school is required on request to provide this form or the required information only for students admitted or enrolled at the school. Throughout this Applicant Self-Certification, “you” and “your” refer to the applicant who is applying for the loan. The applicant and the student may be the same person.
